肺癌の経過中に生じた呼吸不全の原因としてpulmonary tumor thrombotic microangiopathy (PTTM) の関与が疑われた1剖検例

Abstract

pulmonary tumor thrombotic microangiopathy (PTTM) は悪性腫蕩の患者で急激な呼吸器症状の悪化をきたす疾患として報告されている。我々は深部静脈血栓症を契機に造影CTおよびPET-CTから原発性肺癌（TlbN3Ml, stage IV） と画像的に診断された70歳男性の剖検でPTTMを認めた症例を経験した。患者は深部静脈血栓症に対する抗凝固療法施行中に, 亜急性に呼吸状態および胸部画像所見が悪化した。抗菌薬投与とステロイドパルス療法を施行したが効果は得られず, 呼吸不全で死亡した。剖検では左肺上葉の肺腺癌であり, 肺末梢肺動脈内には腫蕩塊を伴う器質化した血栓や肺動脈壁の線維細胞性内膜肥厚, 内腔の狭小化が散見され, pulmonary tumor thrombotic microangiopathy (PTTM）が認められた。本症例のように, 担癌患者において呼吸器症状の悪化を認める場合には, 原因検索として急速に進行をきたすPTTMを鑑別においた早急な画像診断が必要と思われる。

Pulmonary tumor thrombotic microangiopathy (PTTM) is a specific type of pulmonary arterial tumor embolism. It is characterized by progressive respiratory failure in patients with metastatic carcinoma. We present a 70-year-old man who was diagnosed as lung cancer (T1bN3M1, stage IV) by CT and PET-CT during screening for deep vein thrombosis. Deterioration (progressive respiratory failure with worsening CT films) had been evident during treatment by anticoagulant therapy for deep vein thrombosis. He died 28days after admission, despite corticosteroid and antibiotics treatment. Postmortem examination revealed PTTM associated with lung adenocarcinoma, which was characterized by widespread fibrocellular intimal proliferation of small pulmonary ateries and arterioles, tumor embolism, and organization and recanalization of thrombosis. Hence, PTTM should be considered in the differential diagnosis of patients with metastatic cancer who exhibit progressive respiratory failure.
