抄録 | 近年, 腰椎椎弓根スクリューの新しい刺入軌道としてcorticalbone trajectory(CBT)が報告された。スクリューと皮質骨の接触面積を最大限に得られることが特徴であるが, その生体力学的研究は十分になされていないのが現状である。今回, 有限要素法を用いて, CBT法により挿入した椎弓根スクリューの固定性を評価した。30例の腰椎CTデータより第4腰椎の3次元椎体モデルを作製した。まず, CBTを含めた5種の異なる刺入軌道を設定し, 引き抜き強度の最も高い軌道を調査した。次に, 従来軌道とCBTの固定性を比較した。スクリュー単体の固定性の評価として, 引き抜き強度 (N), 多方向荷重に対するスクリューの制動性 (N/mm) を比較した。さらに, 各種負荷に対する椎体の固定性として, 屈曲・伸展・回旋・側屈モーメントに対する椎体制動性(N/mm)を検討した。結果は, 5種の刺入軌道の中で, CBTの引き抜き強度が最も高かった。CBTは従来軌道と比較して, 引き抜き強度は平均26.4%高く, スクリューの制動性は, 頭尾側方向の負荷に対し平均27.8%, 内外側方向に対し平均140.2%高かった。椎体制動性は, CBTは従来軌道に比べ, 屈曲は51.0%, 伸展は35.0%高かったが, その一方で, 側屈は19.6%, 回旋は37.3%低かった。本研究はCBTの固定性を多面的に評価した初めての報告であり, 脊椎疾患に対するCBTを用いた新しい治療戦略の確立に大きく貢献するものと考える。 CBT is a novel lumbar pedicle screw trajectory which follows a cranio-laterally directed path. While CBT takes advantage of maximizing the engagement with cortical bone, little is known about the biomechanical strength using CBT. The purpose of the present study was to evaluate quantitatively the anchorage performance of CBT by the finite element (FE) method. Thirty FE models of lumbar vertebrae were computationally created and pedicle screws were placed using traditional trajectory (TI) and CBT. First, the fixation strength of a single screw was compared by the axial pullout and multi-directional loading tests. Next, vertebral fixation strength of a paired-screw construct was examined by applying forces simulating flexion, extension, lateral bending, and axial rotation to the vertebra. CBT demonstrated 26.4% higher pullout strength, 27.8% higher strength in cephalocaudal load, and 140.2% higher strength in mediolateral load than TT. CBT construct showed higher vertebral fixation strength in flexion (51.0%) and extension (35.0%) and lower vertebral fixation strength in lateral bending (19.6%) and axial rotation (37.3%) compared with TI construct. This study has clarified the fixation strength using the CBT technique, and the results are beneficial for spinal surgeons in the selection of operative methods. |
